Bridgetown Kids | Classroom Support Form

We want Bridgetown Kids to be a space of belonging for every child. The questions below are entirely optional and are intended to help our team better understand how to care for your child during Sunday Gatherings.

You are welcome to share any information about medical needs, disabilities, learning differences, behavioral supports, or accommodations that may help your child participate successfully. You are not required to share official school, medical, or diagnostic documents (such as IEPs or 504 plans), or anything beyond what you are comfortable with sharing.

Any information you choose to provide will be kept confidential and shared only with our staff or volunteers who need it in order to support your child appropriately.
